Summary
Background
Serial dopamine transporter (DAT) imaging in patients with Parkinson's disease (PD) and other synucleinopathies shows progressive nigrostriatal dopaminergic dysfunction. Because idiopathic rapid-eye-movement (REM) sleep behaviour disorder (IRBD) can precede the classic symptoms of PD and other synucleinopathies, we postulated that serial DAT imaging in patients with IRBD could be used to detect decline in striatal tracer uptake, indicating progressive nigrostriatal cell degeneration.

Methods
In a prospective study, 20 patients with IRBD (mean age 70·55 years [SD 6·02]) underwent serial DAT imaging with 123I-2β-carbomethoxy-3β-(4-iodophenyl)-N-(3-fluoropropyl)-nortropane (123I-FP-CIT) SPECT at baseline and again after 1·5 years and 3 years; 20 age-matched and sex-matched control participants (69·50 years [6·77]) underwent imaging at baseline and 3 years. The striatum to occipital cortex uptake ratios were calculated for the putamen and caudate nucleus in each hemisphere. In patients, the ratio was judged to be reduced when it was less than two SD of the mean ratio in controls at the same timepoint. Differences in 123I-FP-CIT uptake between patients and controls in each striatal region and rates of decline were assessed by use of multivariate ANOVA (MANOVA).

Findings
Compared with controls, patients had significantly reduced mean 123I-FP-CIT binding in all four striatal regions at baseline and after 3 years. Striatal 123I-FP-CIT uptake was reduced compared with that in controls in ten patients at baseline and in 13 patients after 3 years. In patients, the mean reduction in 123I-FP-CIT uptake from baseline to 3 years was 19·36% (95% CI 15·14 to 23·59) in the left putamen, 15·57% (10·87 to 20·28) in the right putamen, 10·81% (6·49 to 15·18) in the left caudate nucleus, and 7·14% (2·74 to 11·56) in the right caudate nucleus. After adjustment for the baseline 123I-FP-CIT uptake ratios, the decline in 123I-FP-CIT binding at baseline to 3 years was significantly greater in patients than in controls in the left putamen (9·78% difference between groups, 95% CI 3·22 to 16·32), right putamen (5·43%, 1·99 to 12·86), and left caudate nucleus (8·07%, 1·44 to 14·70), but not in the right caudate nucleus (4·16%, −3·00 to 11·34). At the 3-year assessment, three patients were diagnosed with PD. These patients had the lowest 123I-FP-CIT uptake at baseline and a mean reduction in 123I-FP-CIT uptake at 3 years of 32·81% in the left putamen, 30·40% in the right putamen, 26·51% in the left caudate nucleus, and 23·75% in the right caudate nucleus.

Interpretation
In patients with IRBD, serial 123I-FP-CIT SPECT shows decline in striatal tracer uptake that reflects progressive nigrostriatal dopaminergic dysfunction. Serial 123I-FP-CIT SPECT can be used to monitor the progression of nigrostriatal deficits in patients with IRBD, and could be useful in studies of potential disease-modifying compounds in these patients.

El 45% de los pacientes que presentan esta dolencia desarrollan la enfermedad de Parkinson y otras enfermedades neurodegenerativas en las que se produce una falta de dopamina en el cerebro.

El 45% de los pacientes con trastornos de la conducta durante el sueño REM desarrollan la enfermedad de Parkinson. Sobre este dato, contenido en un estudio de 2006, se han levantado otros dos, todos ellos llevados a cabo por la Unidad Multidisciplinar de Trastornos del Sueño y del Servicio de Neurología del Hospital Clíni-IDIBAPS. El tercero de ellos, publicado en Lancet Neurology, propone el SPECT cerebral, una técnica de neuroimagen, como primera herramienta para visualizar de forma precoz la progresión de la enfermedad.

El primer trabajo demostraba en 2006 que el 45% de los pacientes que lo padecen desarrollaban la enfermedad de Parkinson y otras enfermedades neurodegenerativas en las que hay una falta de dopamina en el cerebro. El segundo artículo señalaba en 2010 que las pruebas de neuroimagen que miden la dopamina en el cerebro, como el SPECT cerebral, permiten identificar los enfermos con trastorno de sueño REM que tienen mayor riesgo de desarrollar precozmente una enfermedad neurodegenerativa como el Parkinson.

Ahora, el nuevo trabajo concluye que el SPECT cerebral detecta cómo los niveles de dopamina en el cerebro bajan rápidamente con el paso de los años en los pacientes con trastorno del sueño REM. Esta técnica de neuroimagen se convierte, así, en la primera herramienta para visualizar de forma precoz la progresión de la enfermedad. Todos los trabajos tienen como primer autor al Dr. Álex Iranzo, médico del Servicio de Neurología del Hospital Clínic de Barcelona, investigador del Institut d’Investigacions Biomèdiques August Pi i Sunyer (IDIBAPS) y miembro de la Unidad Multidisciplinar de Trastornos del Sueño, y como últimos firmantes al Dr. Joan Santamaría y el Dr. Eduard Tolosa, de la misma institución.

El estudio ha consistido en comparar la evolución del SPECT cerebral de 20 pacientes con trastorno de la fase REM y 20 personas control durante tres años. La técnica de neuroimagen consiste en medir la presencia de dopamina en la sustancia nigra, una parte del cerebro relacionada con el aprendizaje y la armonía de los movimientos. En la enfermedad de Parkinson existe un déficit de dopamina en la sustancia nigra y por ello los pacientes tienen temblor, rigidez y lentitud al moverse.

Fármacos neuroprotectores

Los resultados obtenidos demuestran que durante los 3 años de seguimiento la producción de dopamina en el grupo control se reduce como consecuencia de la edad en un 8% y en el grupo de pacientes con trastorno del sueño REM en un 20%. Una vez finalizado el seguimiento de 3 años, 3 de los 20 pacientes del grupo con trastorno del sueño REM ya habían desarrollado la enfermedad de Parkinson y su déficit de dopamina era del 30%.

De los tres trabajos encabezados por el equipo del IDIBAPS-Hospital Clínic se desprende que hay que centrar los esfuerzos en la creación de fármacos neuroprotectores que eviten la progresión de los pacientes con trastorno de la conducta durante el sueño REM hacia enfermedad de Parkinson. Por primera vez se dispone de una técnica, el SPECT cerebral, para evaluar si estos medicamentos son eficaces. Los autores del estudio proponen que para ser eficaz un fármaco neuroprotector debería evitar que la concentración de dopamina se redujera más allá del 8% con el paso del tiempo.
